Your information is private and not accessible to anyone at any time, other than the instructor for whose class you are registering. If the instructor determines that you do not meet the requirements, your credit card will be automatically refunded.

Cancellation Policy

Any cancellation or rescheduling made by Urban Survivor will be fully refunded to the student. Cancellations reasons include severe weather or instructor illness. We will be training in inclement weather and absent active lightning the class will proceed. Cancellations made by the student 5 (five) full days prior to the start of class will be fully refunded minus an enrollment processing fee. Cancellations made by the student with less than 5 days’ notice will be offered a rescheduled class as availability permits. Cancellations or no shows with less than 24 hours from the start of class will be forfeited.

An enrollment can be substituted for another person with prior authorization from the instructor.

FIREARMS TRAINING AGREEMENT
RELEASE AND WAIVER OF LIABILITY

URBAN SURVIVOR LLC AND ITS INSTRUCTORS WILL PROVIDE TRAINING AND INSTRUCTION ON THE BASIC USE OF A FIREARM AND TOPICS ASSOCIATED WITH LAWFUL USE, TRANSPORTATION, AND STORAGE OF A FIREARM. CONCEALED CARRY ISSUES ARE A LEGAL MATTER AND PARTICIPANTS SHOULD SEEK OUT THE ADVICE OF AN ATTORNEY OR A COUNTY OFFICIAL IN CHARGE OF CONCEALED WEAPONS CARRY. THE INSTRUCTORS AND URBAN SURVIVOR LLC WILL NOT PROVIDE LEGAL ADVICE OR OPINION. THE INSTRUCTORS ARE NOT REPRESENTING ANY LAW ENFORCEMENT AGENCY.

I hereby assume all risks by agreeing to the following:

That I am voluntarily participating in a class from Urban Survivor LLC and do so of my own free will. I understand the risks and hazards involved, and I recognize serious and life-threatening injuries could result from this training. I also understand that it is my responsibility to consult with a physician prior to and regarding my participation in class. I represent that I am physically fit and I have no medical, psychological, or any other condition which would prevent my full participation in the firearms training.

In consideration of being permitted to participate in the activities of the firearms training, I agree to assume full responsibility for any and all risks, injuries, or damages, known and unknown, which I might incur as a result of particiating in the class. I further hold Urban Survivor LLC and its instructors harmless for any actions taken by me now or hereafter relative to the use (legal or illegal) of firearms under any circumstances.

I certify that I am at least twenty-one (21) years old and I am legally entitled to own and use a handgun and/or firearm. I further certify I have no felony criminal record, conviction, or sentence in Pennsylvania or any other state and I am not a fugitive from justice, a wanted person, or have been adjudicated mentally incompetent. If I am under the age of 21, my parent or guardian shall sign this waiver on my behalf and be shall be present during the entire class.

I agree that if any provisions of this release are held to be invalid, nevertheless, the balance of the release shall continue in full force and effect.

I agree that if the instructors believe that I am or will be involved in criminal activity, the instructors may at their discretion choose to discontinue training without refund. I further agree to hold the instructors and Urban Survivor LLC harmless for libel or slander as a result of that determination. I agree that if the instructors believe that I am or will be unsafe or dangerous, they can stop instruction with no or a partial refund.

I understand that I must sign this release of liability and fully comprehend the structure and nature of the trainings, and also the risks involved before being allowed to participate in this training. I have read the above Release and Waiver of Liability and fully understand its contents. I have been informed of all risks involved and I voluntarily agree to the terms and conditions stated above.
