Spartan Survival Systems was created in 2013 to help teach families and individuals how to preapre for major disasters and survive dangerous situations.  We have been all over the country since then and now, we are in Northeast Ohio with our indooor tactical trainng facility.  Our instructors have many years of real work experince and thousands of hours of taining, and we are happy to pass that training on to you. Our back round comes from Military comabt to SWAT operations, and everything in between.  So wheter you are looking for hand to hand self defense, or advanced weapons training, we have a course for you.  Train with us at our facility, or we can come to you.

Release of Liability Form

 

Please read this Release of Liability Form carefully before participating in any activities or programs provided by Spartan Survival Systems. By signing this form, you acknowledge and agree to the terms outlined below.

 

Assumption of Risk:

I understand that participation in the training activities and programs offered by Spartan Survival Systems involves certain inherent risks and dangers, including, but not limited to, physical exertion, contact with other participants, potential exposure to natural elements, and the use of tools and equipment. I acknowledge that these risks may result in personal injury and/or property damage. I voluntarily assume all such risks associated with my participation in the activities and programs.

 

Health and Fitness:

I represent that I am physically and mentally capable of participating in the activities and programs offered by Spartan Survival Systems. I have consulted with a medical professional and received clearance to participate if I have any health concerns or pre-existing medical conditions.

 

Release of Liability:

In consideration for being permitted to participate in Spartan Survival Systems' activities and programs, I, on behalf of myself, my heirs, and my personal representatives, hereby release, waive, discharge, and covenant not to sue Spartan Survival Systems, its owners, employees, agents, representatives, successors, and assigns from any and all claims, liabilities, actions, damages, costs, or expenses arising out of or in any way connected with my participation, including but not limited to, any negligence or omission of Spartan Survival Systems.

 

Indemnification:

I agree to indemnify and hold harmless Spartan Survival Systems, its owners, employees, agents, representatives, successors, and assigns from and against any and all claims, liabilities, actions, damages, costs, or expenses, including attorney's fees, arising out of or in any way connected with my participation in Spartan Survival Systems' activities and programs.

 

 

Media Release:

I grant Spartan Survival Systems the irrevocable right and permission to use any photographs, videos, or other media taken during my participation in their activities and programs for promotional, advertising, or any other purpose without compensation.

 

Severability:

If any provision of this Release of Liability Form is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.

 

I HAVE CAREFULLY READ THIS RELEASE OF LIABILITY FORM AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT BY SIGNING THIS FORM, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE SPARTAN SURVIVAL SYSTEMS.
